# QAPI

> Quality Assurance and Performance Improvement initiatives and team assignments.

QAPI holds your Quality Assurance and Performance Improvement initiatives and the teams running
them.

QAPI is a regulatory expectation, not an optional programme. Surveyors look for evidence that your
agency identifies problems, acts on them, and checks whether the action worked.

**Follow-up** is the field that distinguishes a real initiative from a documented intention. An
initiative that closed without follow-up does not demonstrate improvement.

  An initiative with no recorded follow-up is the most common QAPI finding at survey. Closing an
  initiative means showing what changed, not just that the work ended.

Each initiative needs named people. An initiative with no team is unowned.

## Running a QAPI initiative

    Set the title, type, and a realistic start and end date.

    Add members on the **QAPI Team** tab.

  <Step title="Act on the problem">
    The work happens in the module where the problem lives.
  </Step>

  <Step title="Record follow-up">
    Measure whether the problem reduced. This is the part surveyors ask about.

  <Step title="Close with evidence">
    Set the status once follow-up shows the outcome.

QAPI works best when initiatives come from data you already hold:

  Pick fewer initiatives and follow them through. Three closed initiatives with measured follow-up
  demonstrate a functioning QAPI programme better than a dozen open ones.

QAPI activity is usually reported to your governing body. Meeting records and attendance live in

## Troubleshooting

| Problem                                     | What to check                                                                                        |
| ------------------------------------------- | ---------------------------------------------------------------------------------------------------- |
| An initiative has no team                   | Add members on the **QAPI Team** tab.                                                                |
| An initiative shows closed but incomplete   | **Follow-up** is blank. Record the outcome before closing.                                           |
| I cannot show improvement at survey         | Follow-up needs a measure, not a narrative. Choose initiatives where you have data.                  |
| QAPI initiatives are not linked to findings | The link is conceptual, not automatic. Reference the finding in the initiative title or description. |
