Forms requires the Professional plan or higher. On Basic it does not appear in the sidebar at
all. See Plans and modules.
Who can use it
Organization Admins, Location Admins, Clinical Managers, Client Coordinators, and Finance Managers, on Professional or Enterprise.The forms list
Each form in the list shows:
An empty list shows “No forms yet. Create your first form to get started.”
What you can do with a form
Edit
Change the form’s fields, layout, and branding.
Fill
Complete the form inside OfficeRidge as a signed-in user.
Produce a paper copy for wet-signature workflows.
Submissions
Review everything collected on that form.
Sharing a form publicly
Published forms have a public link built from the form’s URL slug. Someone with that link can fill in or print the form without signing in to OfficeRidge — useful for referral sources, families, and applicants. Only published forms are reachable this way. Create the form and name it
The title is what staff see in the list and what recipients see at the top of the form.

Add your fields
The Fields count in the list reflects what you add here.

Set the URL slug
This becomes the public address. Choose something stable — changing it breaks links you have
already shared.

Publish
Until the form is published, the public link returns nothing.

Collect and review
Responses land in Submissions for that form.
