Building a care plan

Start from the assessments
A plan should follow the member’s assessment findings. See
Assessments.

Create the plan and assign the physician
Set the plan name, start and end dates, and the attending physician.

Set the next review date
Next Review keeps the plan in your review cycle. A plan without one drifts out of date
unnoticed.

Activate it
Move the plan from
DRAFT to ACTIVE once it is agreed.5
Document against it
Visit notes accumulate on the Visit Notes tab as care is delivered.

Check what is due
Filter Supervisory Visits on Next Due to find members approaching or past their interval.

Conduct and record the visit
Record the visit date and mark it completed.

Confirm the next due date advanced
Next Due should move forward. If it has not, the visit was not recorded as complete.
