QAPI holds your Quality Assurance and Performance Improvement initiatives and the teams running them. QAPI is a regulatory expectation, not an optional programme. Surveyors look for evidence that your agency identifies problems, acts on them, and checks whether the action worked.

An initiative that closed without follow-up does not demonstrate improvement.
An initiative with no recorded follow-up is the most common QAPI finding at survey. Closing an initiative means showing what changed, not just that the work ended.

Running a QAPI initiative

Identify the problem from evidence

Recurring audit findings, complaint patterns, and incident clusters are the best sources. See Quality and audits.

Create the initiative

Set the title, type, and a realistic start and end date.

Assign the team

Add members on the QAPI Team tab.

Act on the problem

The work happens in the module where the problem lives.

Record follow-up

Measure whether the problem reduced. This is the part surveyors ask about.

Close with evidence

Set the status once follow-up shows the outcome.

Choosing what to work on

QAPI works best when initiatives come from data you already hold:
Pick fewer initiatives and follow them through. Three closed initiatives with measured follow-up demonstrate a functioning QAPI programme better than a dozen open ones.
