Employee health records cover the occupational health screening your staff need before and during member contact.
These records contain employee health information. Restrict access to staff with a genuine need and handle them with the same care as member information.

TB Symptom Surveys

TB symptom screening surveys with completion status and attachments. Columns: Employee · Survey Date · Status · Symptoms A recorded symptom is a prompt for clinical follow-up, not just a data point. Act on it before the employee’s next assignment.

TB Clearances

TB test clearance records with test type, results, and next screening dates. Columns: Employee · Test Type · Test Date · Status · Next Screen Next Screen drives the screening cycle. Filter on it to find staff coming due.
An employee whose TB clearance has lapsed should not be delivering care. Track Next Screen the same way you track credential expiry.

Hepatitis Consents

Employee hepatitis B vaccination consent and declination records. Columns: Employee · Consent Type · Date · Vaccination Date Consent Type distinguishes consent from declination. Both are valid outcomes and both must be recorded — a declination on file is what demonstrates the offer was made.
